Inclusion criteria

Inclusion criteria: Inclusion criteria: Age over 60 years; Have COPD; Ability to do comprehensive care programs; Non-acute mental disease; Lack of mental disorders; learning ability; ?Access to telephone; Be at least five days in the hospital Exclusion criterion:Non-compliance; Death; Failure to leave the patient from acute illness
