21 Years to 80 Years

Inclusion criteria

Inclusion criteria: Inclusion criteria: Age 21 to 80 years ? According diagnosed with heart failure, ejection fraction less than 35% ? -an underlying disease, type 2 diabetes and oral diabetes medications ? capability of communication and literacy and the lack of cognitive disabilities ? Not having renal failure and diabetic eye complications ?Not having kidney failure and diabetic eye complications ?History of hospitalization due to heart failure over time?no Participation in similar educational studies ? Non-membership in medical groups exclusion criteria: absence in more than one training sessions or counseling for groups ? no willing to continuation of corporation in study for both groups?Patients that suffer from acute condition during the intervention or pass away or get involve in severe physical disability ? change of drug regime for diabetes and heart failure.
