Inclusion criteria

Inclusion criteria: Inclusion criteria: patients with acute and chronic heart disease; stable condition; age between 18-60 years; the desire to participate in the research; ability to read and write; access to the phone after hospital discharge. Exclusion criteria: patient's unwillingness to continue to cooperate; exacerbation and incidence of disabilities or life-threatening conditions.
