Inclusion criteria

Inclusion criteria: Age between 40 and 75 years Being married Diagnosis of metabolic syndrome with at least three of the criteria defined by the American Heart Association (AHA), including: a.Dysglycemia (fasting plasma glucose >100 mg/dL (5.6 mmol/L), or random blood glucose >200 mg/dL, or receiving pharmacological treatment for hyperglycemia). b.Elevated plasma triglycerides (=150 mg/dL (1.69 mmol/L) or treatment for hypertriglyceridemia). c.Low HDL cholesterol (<40 mg/dL (1.03 mmol/L) in men). d.Hypertension (blood pressure =130/85 mmHg or treatment with antihypertensive medications). e.Waist circumference =102 cm in men Willingness to participate in the study and completion of the informed consent form Adequate psychological and physical readiness to participate in the study and accept the intervention Orientation to time, place, and person Ability to communicate effectively Familiarity with and comprehension of the Persian language (or local dialect at the study site) Access by the patient or a first-degree household member to a mobile phone Residency in Khorramabad city or its suburbs

Exclusion criteria

Exclusion criteria: Death of the participant during the study Diagnosis of acute cardiac conditions such as heart failure or myocardial infarction (MI) Hemodynamic instability Loss of contact with the participant, including change of phone number or address without notifying the researcher Occurrence of life-threatening events such as myocardial infarction, hypertensive crisis, or diabetic ketoacidosis History of bariatric surgery within the past two years Undergoing surgical, hormonal, or pharmacological treatments for fertility enhancement during the intervention Presence of psychiatric or neurocognitive disorders such as dementia or Alzheimer’s disease Severe hearing, visual, or speech impairments Participation in other lifestyle modification programs
