1- Intervention group ( Short Message Service follow up): after recruiting the patients will receive SMS 3 times per week in the first month and two times per week in second and third month. the SM
2- Intervention groups (telephone follow up ): the researcher will call to patients regularly and educate about disease, sign & symptom, treatment adherence and educations about their conditions.
3- Control group: the patients will receive regular care.

Inclusion criteria

Inclusion criteria: Inclusion criteria : using cell phone and Short Message Services; completing written informed consent; literacy; no blind and deafness; class 2&3 heart failure based on AHA; CHF and EF less than 40%; not having malignancies, diabetes, renal failure; and age range between 35-65 years. Exclusion criteria: not interesting for continuing; imposed disease such as pneumonia; death and cardiac implant; surgery and intervention.
