Chronic disease self management program is a standard program that is based on the self-efficacy theory of Bandura .This Program for six weeks and one class per week for two and a half hour per week

Inclusion criteria

Inclusion criteria: Inclusion criteria:having the ability to reading and writing; under 70; OK a certificate specialist physicians based general health and ability to participate in program; Not engage in other self-management programs. Exclusion Criteria: absence of more than two sessions of class; not wanting to continue attending classes.
