Interventions

Intervention 1: we administer intravenous dexametazon (8mg/every8h) for Patients who were in Group 1 from 24 hours befor extubation to 24 hours after extubation. Intervention 2: we administer Bodeson
Treatment - Drugs
we administer intravenous dexametazon (8mg/every8h) for Patients who were in Group 1 from 24 hours befor extubation to 24 hours after extubation
we administer Bodesonide by aerosolized nebuliser(0.5mg/every6h) for Patients who were in Group 2 from 24 hours befor extubation to 24 hours after extubation

Inclusion criteria

Inclusion criteria: Inclusion criteria: patients' age 18-55 years old; intubation duration more than 48 hours and negative cuff leak test, Exclusion criteria: use of corticosteroids; history of chronic cardiopulmonary diseases, Diabetes mellitus, Gastro intestinal ulcers; patients who reintubated after extubation for other reasons such as: respiratory failure due to muscle weakness, massive airway secretions, sepsis and GCS<8.
