Conditions

arrhythmia, Heart failure. Persistent atrial fibrillation

Interventions

Intervention 1: Intervention group: Patients who are taking 0.125 mg digoxin. Intervention 2: Control group: The same patients will cross over to 0.25 mg digoxin per day except Mondays and Fridays.

Inclusion criteria

Inclusion criteria: Patients under treatment with digoxin for atrial fibrillation or heart failure, who were on a fixed regimen for at least two months prior to the trial and there were no need for any changes to the medications for any reason.

Exclusion criteria

Exclusion criteria: 1- were hospitalized during the course of the trial because of pulmonary edema, worsening of the dyspnea or heart failure decompensation; 2- Needed any changes in other medications such as diuretics, beta-blockers because of worsening of symptoms or other reasons during the trial; 3- Had serum creatinine of more than 1 mg/dl or its increase to more than 1 mg/dl during the trial; 4- Suffered from chronic obstructive pulmonary disease (COPD); 5- Experienced considerable electrolyte imbalance6- Were not able to tolerate the medical regimen because of nausea, vomiting or other Digoxin intoxication symptoms

Heart rate. Timepoint: 1 month. Method of measurement: Physical exam.;Palpitation score. Timepoint: 1 month. Method of measurement: 7 point visual score.;Dyspnea score. Timepoint: 1 month. Method of measurement: 1 to 7 point visual scoring system.
