Inclusion criteria
Inclusion criteria: Inclusion criteria: All COPD (Chronic Obstructive Pulmonary Disease ) and ASA II (American Society of Anesthesiologis )patients that are under mechanical ventilation inpatient 501 hospital. Exclusion criteria:patients with gastric problems and patients with hypotention (SBP under 90 mmHg) during drug prescription.

| Rapid Shallow Breathing Index [RSBI]) = RR / TV. Timepoint: At the end of one week treatment with sildenafil. Method of measurement: To calculate the RSBI at the end of the study, the ventilator is adjusted on self respiratory order and the following tidal volume and respiratory frequency per minute respiration with maximum frequency are calculated.;Plateau Pressure or Alveolar Pressure. Timepoint: At the end of one week treatment with sildenafil. Method of measurement: To calculate the plateau pressure while the patient is under mechanical ventilation and received a constant tidal volume, a temporary occlusion maneuver of 20 seconds duration at the end of expiration with an inspiration pause at the beginning of next inspiration is introduced.;Mixed venous oxygen pressure (P- vO2). Timepoint: At the end one week treatment with sildenafil. Method of measurement: According to Fick’s equation, the rate of P- vO2 is an index of heart output (QT), so oxygen pressure of venous blood (P- vO2) may be calculated by a sample of venous blood collected by central venous catheter. | — |
