Inclusion criteria

Inclusion criteria: Inclusion criteria: 1) GCS=15 2) patients having COPD or CHF 3) Giving consent Exclusion criteria: 1) patients undergoing surgery 2) Psychological problems 3)Those using psychotropic drugs 4) Those not giving consent 5) patients in coma 6) patients having memory or speach problems

Severity of disease. Timepoint: Everyday. Method of measurement: with sofa and apache score.
