Inclusion criteria

Inclusion criteria: - Adults (aged 18–65) who are spending a 6-week therapeutic stay at the Alpreflect clinic in Hopfen am See - Existing diagnosis of (1) recurrent depressive disorder, currently in a moderate episode, or (2) a first-time, moderate depressive episode - Ability to understand the study information and provide written consent to participate in the study

Exclusion criteria

Exclusion criteria: - medical contraindications for Kneipp treatments (e.g. severe cardiovascular diseases) - insufficient language skills that make communication and informed consent impossible - that make communication impossible - lack of email access (access is required to take part in online surveys)

Design outcomes

Primary

MeasureTime frame
Changes in the severity of depression, as measured by the Mini SCL and BDI-II at the start and end of treatment, and 3 months later

Secondary

MeasureTime frame
1. Patient satisfaction, measured using the DRV feedback questionnaire at discharge 2. Physiological parameters during the treatment stay, based on nasal secretions, 24-hour urine collection and a smartwatch (subgroup of 180 participants, up to the first follow-up survey after 3 months) 3. Sustained improvement in psychological distress measured using the Mini SCL, BDI-II Environmental Awareness in Germany 2024 (modified), ERI-S and SF-8, SpREUK-15, ASKU 6 months before, and 6 and 9 months after therapy
