Inclusion criteria
Inclusion criteria: Inclusion criteria for patients: - Age = 65 years - Geriatric-typical multimorbidity (= 2 chronic conditions) - Residing in or around Hoyerswerda - General practitioner referral for Mobile Geriatric Rehabilitation - Fulfilment of MoGeRe indications - Capacity to provide informed consent Physiotherapists as practitioners: Three state-certified physiotherapists from HOY-REHA GmbH are involved in the study. All have experience in treating geriatric patients and have previously conducted conventional Mobile Geriatric Rehabilitation. They are not recruited based on predefined eligibility criteria but are involved in the study as practitioners delivering the intervention.
Exclusion criteria
Exclusion criteria: Exclusion criteria for patients: - Care level 5 (highest level of long-term care dependency in Germany) - No expected physical or cognitive improvement (as assessed by a physician) - Acutely progressive conditions without potential for increased physical capacity - Palliative care orientation - Severe dementia (MMSE < 10)

| The primary endpoint is the feasibility of Blended Physiotherapy within Mobile Geriatric Rehabilitation, assessed through quantitative and qualitative aspects. Quantitative feasibility is evaluated using predefined success criteria across 7 dimensions: recruitment, retention and dropout, treatment duration and frequency, treatment adherence, technical disruptions, safety and adverse events, and data completeness. Assessment is based on a traffic-light system. Data are collected continuously throughout the intervention period (T0 to T1) and at study completion via routine documentation, therapy protocols, and standardised short scales (FIM, AIM, IAM). Qualitative feasibility is assessed through semi-structured individual interviews with patients from the intervention group and all participating physiotherapists following completion of the intervention (T1). The interviews capture experiences, barriers, facilitators, and suggestions for improvement from the perspectives of users and practitioners. | — |

| The secondary endpoint comprises preliminary effectiveness parameters regarding mobility and cognitive performance. Data are collected at two time points: at the beginning of the first therapy session (T0, baseline assessment) and at the last therapy session (T1, post-assessment). Mobility is assessed using the Performance-Oriented Mobility Assessment (POMA) according to Tinetti, and cognitive performance using the Mini-Mental State Examination (MMSE). Depending on the therapy focus, either the POMA (mobility focus) or the MMSE (cognition focus) is administered. | — |
