Inclusion criteria
Inclusion criteria: Incident causally or temporally related to physical therapy care. Incident occurring during or immediately after physical therapy treatment. Physical therapy is an active component of care (primary or clearly documented secondary involvement). Incident involves communication, referral, or handover processes where physical therapy is involved.
Exclusion criteria
Exclusion criteria: Physical therapy is only mentioned without functional relevance to the incident (i.e., no involvement in cause, context, or process). Incomplete datasets (missing core variables, e.g. case description, incident type). Duplicate reports.
