Inclusion criteria
Inclusion criteria: Dyadic Interviews with Patient and Family member/Caregiver: • The patient and a family member/caregiver are able to give their consent for participating data collection. • The patient has had at least one hospitalization with a geriatric indication in the last 6 months. • The patient and the family member/caregiver have sufficient language skills to conduct the interview (interview language: German). • The patient agrees to an expert interview between the researchers with their GP. Expert interviews with GPs: • The GP has treated the patients, who would like to participate in the dyadic interviews, in the last 6 months. • The patient has given their consent for their GP to conduct an expert interview about their medical history.
Exclusion criteria
Exclusion criteria: Dyadic Interviews with Patient and Family member/Caregiver: • The patient and/or their family member/caregiver are not capable of giving informed consent. • The patient has not been hospitalized for geriatric reasons in the last 6 months. • The patient and family member/caregiver do not have sufficient language skills to conduct the interview (interview language: German) • The patient does not consent to an expert interview with their GP. Expert interviews with GPs: • The GP has not treated the patients, who would like to participate in the dyadic interviews, in the last 6 months. • The patient did not give their consent for their GP to conduct an expert interview about their medical history.
