Group 1: Retrospective analysis of all patients who underwent mediastinoscopy at the University Hospital Aachen between 2005 and 2025.

Inclusion criteria

Inclusion criteria: Patients were identified from the Department of Thoracic Surgery database. All patients who underwent mediastinoscopy were screened using the following German procedural (OPS) codes: 1-691.1 – Endoscopic examination of the mediastinum with biopsy (diagnostic mediastinoscopy) 5-342.02 – Mediastinotomy for mediastinal lymph node staging in lung neoplasms Eligible cases were further identified using the following ICD diagnosis codes: C34.* – Malignant neoplasm of bronchus and lung C38.1 / C38.3 – Malignant neoplasm of the mediastinum C77.* – Secondary malignant neoplasm of intrathoracic lymph nodes C81.* – Hodgkin lymphoma C82.* – Follicular non-Hodgkin lymphoma C83.* – Diffuse non-Hodgkin lymphoma C85.* – Other and unspecified non-Hodgkin lymphomas D86.* – Sarcoidosis A15–A19 – Tuberculosis D36.* – Benign neoplasm of the mediastinum or lymphatic system D15.1 – Benign neoplasm of the mediastinum R59.* – Lymphadenopathy, unspecified

Exclusion criteria

Exclusion criteria: Age < 18 years at the time of the procedure. Incomplete medical records Missing essential clinical, operative, or histopathological data required for outcome assessment. Duplicate entries / repeat procedures in the same patient In cases of repeated mediastinoscopy, only the first procedure was included in the primary analysis.

Diagnostic Yield of Mediastinoscopy- The proportion of performed mediastinoscopies that result in a definitive histopathological diagnosis (malignant or benign entity) relative to the total number of procedures performed.
