All patients readmitted from the internal medicine departments within 14 days of the study period. Exclusion criteria: Patients transferred to other hospitals or rehabilitation clinics. Planned readmissions within 14 days (e.g., planned surgeries, scheduled inpatient follow-up appointments).

Determination of the frequency of readmissions within 14 days of discharge from internal medicine departments. Description of patient characteristics (e.g. age, sex, primary diagnosis, other acute diagnoses, comorbidities, length of stay, and place of residence after discharge). Identification of potential associations with readmissions (e.g., age, multimorbidity, discharge diagnosis, polypharmacy, length of stay, and clinical condition at discharge).
