Inclusion criteria
Inclusion criteria: • Geriatric patients admitted to an acute geriatric ward • Sufficient mobility to reach the room designated for the intervention independently or with use of mobility aids, as assessed by ward staff • Barthel Index = 40 • Subjectively capable of participating in the intervention and understanding instructions, according to the clinical assessment of the treating physicians and research staff • Expected to be available by telephone after hospitalization • Consent to audio recording of interviews • Receives at least one module according to the decision algorithm
Exclusion criteria
Exclusion criteria: • Documented diagnosis of dementia • Acute delirium or psychosis • Acute suicidality • Barthel Index < 40 • Diagnosis of a disease that impairs the capacity to consent to participation • Unlikely to be available by telephone after discharge • No consent to audio recording of interviews
