Inclusion criteria

Inclusion criteria: • Traumatic finger amputations (any length) • BG (German Social Accident Insurance) and private context • Primary care either by means of a semi-occlusive film dressing or by means of surgical stump formation (usually local skin flap, but also V-Y plasty, Moberg, Foucher, etc.) • Consent to prospective follow-up examination (for the prospective part) • Age = 18 years

Exclusion criteria

Exclusion criteria: • Replantations (as these are still considered the gold standard for suitable indications) • Partial amputations with initial revascularization intention • Treatment using full-thickness skin grafts, Integra®, or similar • Non-traumatic amputations (e.g., due to infection, tumor, frostbite) • Minors (< 18 years of age)

Primary

MeasureTime frame
Study objectives: 1. Healing process and complication rate: Comparison of the healing time until complete wound closure and the frequency of secondary complications (e.g., infection, secondary interventions, wound healing disorders) between both treatment groups. 2. Functional and sensory results: Objective assessment and comparison of postoperative mobility (goniometer), grip and pinch strength (dynamometer, pinch gauge), and sensitivity (2-point discrimination, Semmes-Weinstein test) between film dressing and stump formation. 3. Pain and neuroma incidence: Investigation of the frequency of neuroma formation and painful nerve endings (Hoffmann-Tinel test, palpation) and comparison of pain at rest and under stress (NRS) between the two groups.

Study objectives: 4. Subjective and aesthetic outcome: Assessment of patient satisfaction (VAS, QuickDASH, aesthetic evaluation, willingness to undergo reoperation) and analysis of the influence of the choice of therapy on perceived hand function and quality of life. 5. Return to work and social reintegration: Analysis of the time to return to work and professional reintegration depending on treatment method, amputation level, and occupational stress (manual vs. administrative). Question: Do amputation level or choice of therapy correlate with the time to professional reintegration? 6. Influence of patient-specific factors on the outcome: Identification of clinical and paraclinical predictors (e.g., diabetes mellitus, PAD, smoking, amputation level, accident mechanism) associated with a favorable or unfavorable functional outcome. 7. Microcirculatory and trophic changes: Objective assessment of thermal and perfusion-related parameters (FLIR camera, HSI measurement) in comparison between treatment groups as possible surrogate markers for wound healing and long-term blood flow.
