Inclusion criteria
Inclusion criteria: Routine data analysis: - All insured persons aged 70 or older with AOK Saxony-Anhalt who reside in Saxony-Anhalt and have had at least one hospital stay in a hospital in Saxony-Anhalt in 2022 Qualitative interviews: - informed consent has been obtained (1) Hospital care providers: Interviewees have experience with discharge management or the care of geriatric patients (2) Follow-up care providers: Interviewees have experience with the transfer of geriatric patients from hospital care to their own facilities or with the care of geriatric patients in general (3) Patients and/or their relatives: (3a) Patients: Interviewees are 70 years of age or older and have had at least one hospital stay in the last 12 months (3b) Relatives: a family member or a similarly close person aged 70 or older has been hospitalized at least once in the past 12 months. The interviewee (relative) was involved in providing support or aftercare.
Exclusion criteria
Exclusion criteria: Routine data analysis: - Insured persons of AOK Saxony-Anhalt, aged 70 and over, with at least one hospital stay in 2022, who died during their hospital stay Qualitative interviews: (1) Hospital care providers: no experience with discharge management or care of geriatric patients (2) Follow-up care providers: no experience with transfers of geriatric hospital patients or care of geriatric patients in general (3a) Patients: younger than 70 years of age and no hospital stay in the last 12 months (3b) Relatives: family member is younger than 70 years of age and has not been hospitalized in the past 12 months; interviewee was NOT involved in providing support or aftercare
