Inclusion criteria

Inclusion criteria: Age = 18 years Evidence of cerebral small vessel disease on MRI (white matter hyperintensities Fazekas grade 1–3) Assignment to one of the three study groups: 1. no acute ischemic stroke 2. acute lacunar ischemic stroke 3. acute territorial ischemic stroke Ability to provide written informed consent Sufficient German language skills to participate in study procedures

Exclusion criteria

Exclusion criteria: Other plausible causes of white matter hyperintensities than cerebral small vessel disease (e.g. inflammatory CNS disorders, leukodystrophies, brain tumors) Known neurodegenerative diseases (e.g. Parkinson’s disease, dementia) Acute or recent traumatic brain injury Contraindications to MRI examination Pregnancy or breastfeeding Life expectancy < 1 year Inability to participate in the study or to comply with follow-up visits

Primary

MeasureTime frame
Correlation of longitudinal changes in blood biomarkers with cerebral small vessel disease progression assessed by serial MRI over 12 months.

Secondary

MeasureTime frame
Comparison of biomarker trajectories between study groups. Association of biomarkers with additional MRI markers (WMH, microbleeds, lacunes, atrophy). Occurrence of cerebrovascular events during follow-up.
