Inclusion criteria
Inclusion criteria: On patient level • Age =70 years. • Smartphone use at least three times per week (Android 6 or higher or IOS 14 or higher). • Sufficient language and cognitive skills to comprehend the study documents. On practice level • Single and group practices. • Licensed for statutory health insurance. • Computers (Windows 7 or higher) with internet access.
Exclusion criteria
Exclusion criteria: On patient level • Acute illnesses or injuries that impair balance, mobility, or the ability to engage in unsupervised home-based physical training (e.g., fractures, amputations). • Neurological disorders associated with pronounced motor limitations, including advanced Parkinson’s disease, advanced-stage multiple sclerosis, severe hemiplegia after stroke, and unexplained syncope. • Severe dementia. • Severe visual or auditory impairments (e.g., blindness, deafness). • Accommodation in a retirement home. On practice level • Exclusively private medical services. • No digital practice management system.
