Inclusion criteria

Inclusion criteria: - Patients who underwent a microvascular free flap reconstruction in the head and neck region at the Department of Oral and Maxillofacial Surgery, University Medical Center Mainz between October 2018 and August 2025. - Availability of complete and relevant routine clinical data, including anesthesia records, operative reports, and postoperative documentation necessary for analysis. - Microvascular free flap procedure performed for reconstructive purposes, such as fibular, radial forearm, anterolateral thigh (ALT), or other established free flap types. - Age = 18 years at the time of surgery. - Elective or standard-of-care reconstructive procedure with documented intraoperative volume therapy parameters (i.e., excluding emergency cases without adequate documentation).

Exclusion criteria

Exclusion criteria: - Incomplete or missing routine clinical data, particularly insufficient documentation of intraoperative fluid therapy or hemodynamic parameters. - Patients younger than 18 years at the time of surgery. - Non-microvascular reconstructive procedures (e.g., local flaps, pedicled regional flaps). - Reconstructions performed outside the head and neck region, even if microvascular.

MeasureTime frame
What: Postoperative survival of the microvascular free flap. When: During the first 5 postoperative days (routine clinical monitoring). How: Evaluation of routine clinical documentation (clinical checks, flap monitoring, indications for revision). A flap is considered “surviving” if no perfusion-compromising event or revision surgery is recorded.

Secondary

MeasureTime frame
Occurrence of arterial or venous perfusion disorders. Postoperative hematoma requiring intervention. Need for revision surgery (categorized by cause). Clinically relevant flap or soft-tissue edema. Partial or complete flap loss. Association between intraoperative fluid therapy (volume/type), vasopressor use, and postoperative complications.
