Group 1: Patients aged 70 and older with an ISAR score of = 2 who are admitted to hospital for surgery are assessed and tested using validated clinical assessment tools covering various geriatric doma

Inclusion criteria

Inclusion criteria: • ISAR (Identifying Seniors at Risk) score = 2 • Inpatient admission for surgery • Surgery has not yet been performed

Exclusion criteria

Exclusion criteria: • Patients receiving palliative care (life expectancy < 3 months based on clinical assessment by the attending physician) • Patients who are unable to give consent and who have no legal guardians or authorized representatives • Assessments cannot be carried out due to limited communication skills (e.g., due to a lack of language skills) • Length of inpatient stay < 3 days

MeasureTime frame
Accuracy of discharge destination decisions made using the SAA compared to expert assessment of the best discharge destination ensuring continuity of care. The following discharge destination options are recorded: • Acute geriatric hospital • Specialized or geriatric rehabilitation (inpatient or outpatient) • Home (with or without personal assistance) • Nursing home

• Independence in activities of daily living before hospitalization and after 3 and 15 months • Care needs before hospitalization and after 3 and 15 months • Mobility before hospitalization and after 3 and 15 months • Readmission rate to Ulm University Hospital (UKU) within 3 months of discharge • Adverse events during hospitalization and in the first 3 months after discharge • Cognition at the time of hospitalization (preoperative) and after 3 months • Occurrence of delirium during hospitalization • Health-related quality of life of participants at the time of hospitalization (preoperative) and after 3 and 15 months • Health economic analysis of health services used within 3 and 15 months after discharge • Exploratory analysis of sensor data collected from the wrist (activity/mobility and feasibility of the measurement method) • Usability and perceived usefulness (quantitative analysis) from the users' perspective, reasons for deviation from COC recommendations
