Inclusion criteria

Inclusion criteria: -At least two diagnosed chronic diseases -The use of at least 3 chronic medications

Exclusion criteria

Exclusion criteria: -Acute mental or physical disease, that may limit study participation -inappropriate knowledge of the German language or limited vocabulary, that prevents interview participation or the expression of detailed answers -Life expectancy shorter than 6 months
