Inclusion criteria

Inclusion criteria: Out-of-hospital cardiac arrest (OHCA) Persistent/refractory cardiac arrest upon arrival in the emergency department, despite guideline-conform prehospital resuscitation efforts Successful establishment of arterial access for invasive blood pressure monitoring in the emergency department No contraindications to tourniquet application on the extremities

Exclusion criteria

Exclusion criteria: Successful ROSC prior to randomization Known clinical or historical contraindications to tourniquet application (e.g., pre-existing severe peripheral arterial occlusive disease, amputations) Severe traumatic injuries to the extremities that preclude safe tourniquet placement
