Inclusion criteria
Inclusion criteria: - ISS > 15 - Trauma-surgical treatment initially largely completed - Severe impairment of activity and participation to be expected - Positive prognosis for achieving the ability to rehabilitate on discharge from post-acute rehabilitation - Patient partially mobilized (2-4 hours wheelchair) - Functional capacity for several therapy services daily for approx. 30 min each - Fractures stable after exercise - Mostly conscious, simple requests can be followed - Able to communicate, interact, if necessary with aids - Statutory health insurance with participating health insurance companies
Exclusion criteria
Exclusion criteria: - Rehabilitation capability given at the end of acute stay or achievable through transitional care in hospital - No benefit commitment from payer - Relevant CNS injuries - Need for intensive medical monitoring/therapy - SHT, paraplegic lesions, vegetative state with indication for neuro-rehabilitation - Delirium - Patients with indication for neurological post-acute or geriatric rehab - Burn patients who take advantage of specialized treatment offers - Suicide risk, risk of self/other endangerment - Manifest addiction or drug addiction - Lack of resilience due to comorbidities - Cost units BG, private cost units or cost units not participating in the project - Not able to communicate, not able to interact - Ground transportation by ambulance not possible - Participation in study refused
