Inclusion criteria
Inclusion criteria: • Insured with one of the participating health insurance companies • Resident in the model region • Complex care needs exist or own care situation is assessed as not good • Consent to participate in the intervention and the associated primary data collection • Availability during participation in the study (12 months)
