Conditions

Participants with lifestyle-related risk factors (exercise, diet, obesity, smoking) and initial signs of disease (conspicuous biomarkers, diabetes, cardiovascular disease, high blood pressure, lipometabolic disorders, chronic back pain, knee/hip osteoarthritis, osteoporosis).

Interventions

Group 1: The study arms differ in the intensity and complexity of the interventions. The participants in study arm 1 only receive a standard programme in the areas of nutrition, exercise and non-smoki

Inclusion criteria

Inclusion criteria: • Adults aged 18 years and older with AOK insurance • Enrolled in the selective contract for family doctor-centred care (HZV) • Lifestyle-related risk factors (physical inactivity, unbalanced eating behavior, smoking) • Elevated biomarkers and/or (first-time) diagnosis of hypertension, lipid metabolism disorders, pre-diabetes, obesity, chronic back pain and hip and knee osteoarthritis

Exclusion criteria

Exclusion criteria: • Insufficient physical capacity (able to walk for at least 20 minutes) • End organ damage • Unable to train on training machines • Insufficient knowledge of German • BMI = 35 kg/m2

Design outcomes

Primary

MeasureTime frame
The primary endpoint is quality of life (QALY = quality-adjusted life years) two years after the start of the study (t24).

Secondary

MeasureTime frame
Secondary endpoints include lifestyle-related risk factors, dietary and exercise behaviour, motivational regulation, fitness, biomarkers, cost of illness (routine data), cost-effectiveness.
