Inclusion criteria

Inclusion criteria: - impairments of the central and/or peripheral nervous system - the existing functional limitation(s) are likely to last longer and are associated with an increased risk of falling in the private home environment and/or require adaptations of the home environment to the patient's functional abilities - unable to walk or only able to walk to a limited extent - discharge to home is planned

Exclusion criteria

Exclusion criteria: - Incapable of giving consent and legal guardianship not assumed by relatives - Existence of conditions that preclude understanding of the intervention goal
