Inclusion criteria

Inclusion criteria: - 70 years of age or older - at least 5 drugs in continuous medication - discharge to home - able to communicate adequately - place of residence within approximately 30 km radius from the study centre to enable a home visit

Exclusion criteria

Exclusion criteria: no further exclusion criteria/non-fulfilment inclusion criteria
