Inclusion criteria
Inclusion criteria: - Geriatric patients over 70 years of age with multimorbidity or over 80 years of age - People being treated in the cardiac surgery, geriatrics or geriatric trauma center at Oldenburg Hospital or - Persons being treated in the orthopaedics or trauma surgery department at Pius Hospital Oldenburg - In addition, if available, a primary contact person/caring relative will be recruited.
Exclusion criteria
Exclusion criteria: - Life expectancy < 6 months - Bedridden - Necessity of permanent use of a wheelchair - Persons living permanently in a nursing home before the inpatient stay - Unstable medical, neurological or psychiatric illnesses - Persons who are unable to understand the contents of the study - Place of residence outside a radius of 40 km around Oldenburg

| Patients and, if available, their relatives will be recruited.relatives may be healthy subjects or people with geriatric characteristics.they will be included in the study in order to investigate the relationship between the activities of patients and relatives as well as the role of the social environment and the stress experienced by caring relatives. The data collection takes place in the hospital and after discharge from the hospital in the participants‘ home environment; as the tests are not carried out for those under 70, they are offered a telephone survey; the participants are visited at three points in time: T0 takes place in the hospital, T1 after 3 months and T2 after 6 months after discharge at the participants’ homes. At each appointment, various assessments and tests will be carried out to determine physical and social activity, health and functional abilities, social resources, stress and support needs as well as gastrointestinal changes. For example, the EQ-5D, Timed up and go, SoS according to Nikolaus, IADL according to Lawton and Brody, Clinical Frailty Scale and the MoCA are used. The recruitment of participants will continue until May 2026, with the last participant visit taking place in November 2026.The statistical analyses obtained will be used to develop an intervention concept by September 2027. | — |
