Inclusion criteria
Inclusion criteria: Able and willing to give consent Age =70 years AND at least 2 of the following criteria: - Degree of care - Mobility impairment - Frailty - Chronic pain syndrome - Cognitive and emotional impairment AND (acute clinical deterioration in the last 6 months OR progressive clinical deterioration within the last 12 months) - Member of the statutory health insurance scheme
Exclusion criteria
Exclusion criteria: - Refusal or inability to participate in the study- Need for inpatient treatment - Completely bedridden - Nursing home resident - Patient is unable to complete a simple questionnaire - Life expectancy less than 8 months
