Inclusion criteria
Inclusion criteria: - Cardiovascular disease at stage I-IV: Respondents should either have a (early) risk to develop a CVD represented by type 1/type 2 diabetes mellitus, and/or other CV risk factors including metabolic disorders and hypertension or have a confirmed CVD, such as CAD, left ventricular hypertrophy, or chronic HF. - Ability to read and understand the questionnaire - Ability to give informed consent - Willingness to participate

Exclusion criteria: - Significant cognitive impairment - Insufficient language skills
