Inclusion criteria
Inclusion criteria: Patients: o Written informed consent of the patient after verbal and written explanation by the attending physician supported by the study staff. o Consecutively all patients treated as inpatients or outpatients in the cardiology department. o Over 18 years of age and fully legally competent. o Resident in their own home or assisted living facility. o Existing long-term medication due to the underlying cardiological disease with at least one drug from ATC group C. o Sufficient knowledge of the German language to conduct the interview. o Patient is supported by a relative in at least one of the following activities: - Accompanying the patient to medical appointments - Accepting medication advice at the pharmacy. - Preparing medication for the patient. - Reminding the patient to take their medication. - Supporting the patient in taking the medication. Caregiver: o Written informed consent of the next of kin after verbal and written explanation by the attending physician supported by the study staff. o Age over 18 years and full legal capacity. o Regular personal contact with the patient. o Involved in the patient's medication process through at least one of the following activities: - Accompanying the patient to medical appointments - Accepting medication advice in the pharmacy. - Preparing medication for the patient. - Reminding the patient to take their medication. - Supporting the patient in taking the medication. o Sufficient knowledge of the German language to carry out the training.
Exclusion criteria
Exclusion criteria: - Involvement of an outpatient nurse in the medication process that goes beyond the following activities: o Weekly or daily administration of medication. o Application of exclusively parenteral forms of medication to the patient. - Patient specific: - Refusal of participation by the patient. - Lack of fulfillment of the inclusion criteria. - Severe cognitive limitations that preclude participation in the intervention and answering the questionnaires. - Communication barriers due to physical limitations (e.g. blindness, deafness) Caregiver specifically: - Refusal of participation by the relative. - Lack of fulfillment of the inclusion criteria. - Severe cognitive limitations that preclude participation in the intervention and answering the questionnaires. - Communication barriers due to physical limitations (e.g. blindness, deafness)
