Inclusion criteria

Inclusion criteria: - Diagnosis of a stroke with hemiparesis, with the last stroke having occurred more than 12 months ago -Age 18-95 years -Remaining mobility of the more severely affected arm: 20° wrist and 10° of each finger -Intact skin on the forearm

Exclusion criteria

Exclusion criteria: -Insufficient knowledge of the German language to interact with study staff -severe cognitive or behavioral problems documented by a physician (e.g. attention deficit, neglect, aphasia, apraxia, dementia) -known allergic reaction to local anesthetic creams -atopic dermatitis -liver diseases (methemoglobinemia prevention)

Shape-sorter-drum test Test (FSBT), main outcome The test subjects have to pick up 20 objects with the affected hand from a standard position and place them in compartments of a drum according to their size and shape. The dependent variable, the performance time in seconds, is measured from the start of the movements until the last object is placed in the drum, so that shorter times represent better performance. The measurement takes place before (t1) and after (t2) the individual occupational therapy on the two test days one week apart.

Grating Orientation Task (GOT), secondary outcome GOT is used to measure the limits of tactile discrimination.We use a set of 14 hemispherical plastic domes with gratings cut into their surface (parallel bars and grooves of equal width [0.5-10 mm]). The ridges and grooves are randomly aligned perpendicular or parallel to the finger axis behind a shield. Subjects are asked to identify the orientation without visual inspection. We determine GOT thresholds, defined as the groove width at which responses are 75% correct. The measurement takes place before (t1) and after (t2) the individual occupational therapy on the two test days one week apart. Independent deafferentation In order to assess the practicability of this use of TD as an additional technique in conventional occupational therapy and for the efficient planning of a follow-up study, it is interesting with regard to a one-hour application time of the cream, to know whether patients will be able to carry out the application independently in the future before the therapy with video instructions. These observations are documented in keywords and then summarized descriptively (frequency of performance without support; identification of obstacles).
