Inclusion criteria
Inclusion criteria: 2. Diagnosed chronic heart disease (e.g. heart failure, CHD, arrythmias) 3. Mental distress (HADS > 12 and/ or diagnosed mental disorder according to ICD-10) 4. Participation in blended collaborative care programm in the psychosomatic outpatient clinic of the University Medicine Göttingen (UMG)
