Combination of mental distress annd chronic heart disease

Interventions

Group 1: We will include 50 psychologically distressed patients with heart disease, who receive a treatment offer in the sense of Blended Collaborative Care in our psychosomatic university outpatient

Inclusion criteria

Inclusion criteria: 2. Diagnosed chronic heart disease (e.g. heart failure, CHD, arrythmias) 3. Mental distress (HADS > 12 and/ or diagnosed mental disorder according to ICD-10) 4. Participation in blended collaborative care programm in the psychosomatic outpatient clinic of the University Medicine Göttingen (UMG)

Exclusion criteria: no consent to participate in the study

1. Adherence of the patients: We want to show that the proportion p of patients who take part in the planned 10 care appointments is not too low. 2. Impact of blended collaborative care under routine conditions on the health-related quality of life measured using the HeartQoL and using a pre-post comparison (t-test) for the time points 4 months and 8 months.	—

Patient preference regarding mode of contact (telephone, video, presence) ? Impact of BCC under routine care on psychosocial parameters such as anxiety, depression, somatization, relationship to treatment team ? Impact of patient characteristics on adherence to and results of the BCC The following questions will be answered qualitatively: How well can the registry from the TEACH study be implemented into routine care? Are any adjustments necessary? How are the workflows for BCC to be designed?	—
