Inclusion criteria

Inclusion criteria: (Partial) inpatient stay in geriatrics, gerontopsychiatry or internal medicine, neurology/geriatric centre, living at home, Geriatric Depression Scale (GDS-15) =4 points, Mini Mental Status Test (MMST) =20 points, multimorbidity, level of care <5, indication for behavioural and physical activation, statutory health insurance, capacity to consent

Exclusion criteria

Exclusion criteria: Living in a nursing home, illness leading directly to death

Stabilisation or improvement of (partial) inpatient treatment outcomes for depression

MeasureTime frame
Improvement of depressive symptoms, reduction of (partial) inpatient days, increase/maintain physical activity, increase/maintain physical functioning, increase/maintain cognitive performance, increase/maintain quality of life, increase/maintain emotional well-being, reduce perceived stress
