Inclusion criteria
Inclusion criteria: Age between 18 and 65 years Observed out-of-hospital cardiac arrest Initiation of conventional CPR measures by bystanders or medical personnel within a maximum of 10 minutes after the event No ROSC (Return of Spontaneous Circulation) after 5-10 minutes of ACLS (Advanced Cardiovascular Life Support) according to the currently valid ERC (European Resuscitation Council) Guidelines Persistent hemodynamic instability despite intermittent ROSC
Exclusion criteria
Exclusion criteria: Treatment refusal by the patient (e.g., existing advance directive) Age 65 years CPR duration > 60 minutes until the arrival of the ECLS team Cardiac arrest due to a non-cardiac cause (e.g., trauma, dissection, bleeding) Initiation of thrombolysis therapy before the arrival of the ECLS team Known severe systemic diseases such as liver cirrhosis Child C, heart failure NYHA IV, or COPD Gold IV Malignant (and incurable) underlying disease Presence of vascular prostheses in the inguinal vessels or abdominal aorta Rejection of the procedure by the attending emergency physician Rejection of the procedure by the patient's attending family members
