Eligibility
Inclusion criteria
Inclusion criteria: • Adult patients 18 years and older • Indication for flexible bronchoscopy with conscious sedation • Written consent • Arterial or capillary blood gas analysis on room air available in last 72 hour before bronchoscopy
Exclusion criteria
Exclusion criteria: • SpO2 or /SaO2 40 kg/m2 • pH < 7.35 .- planned Argon-Plasma or electrocoagulation • Continuous need for oxygen (long term oxygen therapy) • Need for non-invasive ventilation • Use of i.v. vasopressors, inotropes or systolic BP < 90 mm Hg • Myocardial infarction within previous 28 days • Stroke within previous 28 days • Mechanical respiratory support (invasive mechanical ventilation, ECMO) • Epilepsy with seizure within last 12 months • Use of illicit drugs • Comorbidity with estimated life expectancy < 6 months • Dementia or inability to communicate • Lack of informed consent
Design outcomes
Primary
| Measure | Time frame |
|---|---|
| The primary endpoint is need for additional oxygen therapy. The need for additional oxygen therapy is recorded during and up to 1 hour after bronchoscopy and is based on the measurements of pulse oximetry (SpO2 less than 90%) and the failure of previous measures to keep the airways open. (see figure in ch 6)) | — |
Secondary
| Measure | Time frame |
|---|---|
| • Episodes of hypoxemia (SpO2 15 sec) during bronchoscopy • Episodes of severe hypoxemia (SpO2 15 sec) during bronchoscopy • Episodes of hypoxemia (SpO2 15 sec) with hypoventilation ( detected clinically or by etCO2 recording) during bronchoscopy • Duration of oxygen therapy (minutes) after intervention 2 • Oxygen area under the curve (L/min) after intervention 2 • Time on oxygen-therapy after intervention 2 • Sedative dose in mg/kg/min • Examination time • VAS satisfaction of sedation (examiner) • patient comfort (scale) • Minimal SpO2 • Time of SpO2 <90%(minutes) • Number of Complications during bronchoscopy • Use of any respiratory intervention | — |
