Group 1: Neonatal or pediatric ARDS (nARDS/ pARDS) which meets the PALICC-2 or Montreux definition . Need for prolonged invasive mechanical ventilation (>24h). Gestational age >38+0 weeks, bodyweight

Inclusion criteria

Inclusion criteria: Inclusion criteria nARDS/pARDS group: - pediatric patients with respiratory failure (nARDS/pARDS and need for intubation with mechanical ventilation >24h - Weight =3.5 kg and/or chest circumference =36 cm - Consent of the parents to participate in the study

Exclusion criteria

Exclusion criteria: Exclusion criteria nARDS/ pARDS group: - Children with palliative care - Lack of parental consent - Contraindication against the performance of an EIT according to approval (e.g. pacemaker).

Design outcomes

Primary

MeasureTime frame
Identification of the best EIT-PEEP according to the EIT-derived Compliance (C EIT).

Secondary

MeasureTime frame
Secondary outcome measures were as follows: identification of further EIT- indices (global inhomogeneity index [GI] and center of ventilation [COV] as well as measurement of the respirator derived compliance (Cdyn). Furthermore duration of mechanical ventilation, ventilator free days after 28 days, inhospital-mortality.
