Inclusion criteria

Inclusion criteria: Diagnosis of acute cerebral ischemic disease (acute ischemic stroke (ICD-10: I63) or transient ischemic attack (ICD-10: G45), diagnosis of at least one cerebrovascular risk factor, written informed consent to participate in the study.

Exclusion criteria

Exclusion criteria: Cognitive or physical inability to understand one's own health condition or to independently manage and execute secondary preventive measures after the acute ischemic disease, relevant language barrier, guardianship.
