Inclusion criteria

Inclusion criteria: - Patients capable of giving consent after allogeneic stem cell transplantation or CAR T-cell therapy - Age at inclusion = 18 years - Place of residence in the regular catchment area of one of the centers (Dresden, Leipzig, Chemnitz, Leipzig) - Patient owns a smartphone, can operate it and use the video telephony option never use it - Patient agrees in writing to participate in SPIZ - Patient is a member of a statutory health insurance fund

Exclusion criteria

Exclusion criteria: -Patients who had been discharged from hospital after allogeneic stem cell transplantation or CAR T-cell therapy was more than 8 days ago at the time of inclusion - Insufficient language skills for the German-language app and questionnaires used in the project. questionnaires used in the project - Place of residence outside the regular catchment area of the treating clinic (e.g. > 200 km) - Patient cannot be cared for in an outpatient setting - Patient is not a member of a statutory health insurance fund or has not signed the consent form. declaration of consent

Design outcomes

Primary

MeasureTime frame
Disease-specific rehospitalization or death; confirmatory review at month 12 after primary discharge. Descriptive comparisons of frequency differences at months 6, 18 and 24.

Secondary

MeasureTime frame
- Complications (death, infection, GvHD, recurrence of the underlying disease): Frequency as well as over time, duration and frequencies of inpatient stays, frequencies of symptoms (e.g. food intake, weight, temperature, skin changes, bowel movements, neurological symptoms, pain, blood pressure) - Quality of life via questionnaires and answers in app, empowerment via pathway adherence (recording symptoms, medication intake, activities e.g. steps/day) and analysis of interviews - Analysis of interviews with patients and service providers, questionnaires on satisfaction - Health economic evaluation of AOK policyholders: frequency and costs of inpatient stays and outpatient costs, costs of outpatient medication, travel costs, level of care, palliative care, costs of the new form of care
