Inclusion criteria

Inclusion criteria: - Patients who have completed tumor therapy and are currently undergoing cancer aftercare - Completion of tumor therapy = 5 years - Age = 60 years

Exclusion criteria

Exclusion criteria: - Palliative patients or patients with advanced cancer - Patients currently undergoing cancer therapy (except: long-term maintenance therapy such as adjuvant hormone therapy)

- Prevalence of established cardiovascular risk factors (arterial hypertension, diabetes mellitus, hyperlipoproteinemia, nicotine abuse, positive family history) and previous cardiovascular events - Identification of specific risk groups, taking into account age, comorbidities, tumor entity, tumor therapies, cardiovascular risk profile and function - Nutritional behavior (short nutritional screening (SHEIH-23) (under validation); Mediterranean diet questionnaire (MEDAS-FFQ) and malnutrition risk (MNA-FB)) - Physical activity (Physical Activity and Sports Activity Questionnaire, BSA) - Psychological distress (NCCN DT); Depression (PHQ-9); Generalized Anxiety Disorder, module of the Health Questionnaire for Patients (GAD-7) - Scale for General Self-Efficacy Expectations (SES6G) - Health-related quality of life (Health-related-QoL) (SF-12) - Implementation factors (facilitators and barriers), conducting qualitative semi-structured interviews with practitioners, caregivers and patients regarding acceptance and feasibility - Utilization rates of the model consultation hour	—
