Inclusion criteria
Inclusion criteria: - Premature infants with increased socio-medical and/or nursing aftercare needs between 34-37 weeks' gestation - Families with newborns born before the 34th week of pregnancy - Families with newborns with extremely low birth weight (<1000g), other low birth weight (1000 to < 2500g), extreme immaturity (gestational age <28 weeks) - Families with sick mature infants (congenital malformations or perinatally acquired diseases) with socio-medical and/or nursing aftercare needs
Exclusion criteria
Exclusion criteria: - Families with premature infants without increased socio-medical aftercare needs or increased nursing aftercare needs - Families with premature and full-term infants with an increased need for follow-up care in a palliative situation
