Inclusion criteria
Inclusion criteria: Patients with advanced cancer, - who require palliative medical care or who are expected to require such care within the next six months - who are about to be discharged from inpatient to primary care; - and who have agreed to participate in the clinical trial of the European Pal-Cycles project (WP3). The WP3 clinical trial will be conducted in seven European countries (Germany, Great Britain, the Netherlands, Poland, Portugal, Romania and Hungary). Health care professionals: The healthcare staff of the participating departments (medical and nursing staff as well as other professionals who are in contact with the patients in the departments: Patient Admission, Case Management, Social Worker, Psychooncologist and Spiritual Counsellor) will be invited to participate in the study.

Exclusion criteria: Patients unable to give informed consent.
