Inclusion criteria
Inclusion criteria: Presence of a chronic disease that justifies outpatient treatment at Hannover Medical School; medium- to long-term outpatient treatment at one of the selected outpatient clinics of Hannover Medical School
Exclusion criteria
Exclusion criteria: Cognitive and/or physical limitations that prevent participation in the standardised online survey; insufficient knowledge of the German language to answer an online questionnaire; participation in another study
