Inclusion criteria

Inclusion criteria: - Patient is being treated by a participating doctor - Patient receives home visits from a care assistant - Conducted patient information with voluntary and written consent for the pseudonymized recording of health-related quality of life and patient satisfaction as well as for the anonymized recording of the care provided and the processing of the personal address by the responsible assistant or a person named by the assistant to calculate the route - Patient is mobile enough to visit the VaO room (sufficient mobility according to the assessment of the supervising general practitioner) - Sufficient knowledge of German

Primary

MeasureTime frame
The saved working time saved (travel and set-up time) is measured at each visit.

Secondary

MeasureTime frame
The changes in quality of life (SF 12) and satisfaction (ZUV 8) are measured compared to the situation before the care concept was implemented.
