Group 1: Intervention Group: participants undergo professional support via telephone following cardiac rehabilitation (approximately three contacts over three months) and participate in a 5-day, 21.25

Inclusion criteria

Inclusion criteria: Cardiac Rehabilitation after an acute event, pension insurance by DRV Berlin-Brandenburg or DRV Nord; Ages 18 to 62; specific work-related problems assessed via Würzburger Screening (Cut-Off 1 point).

Exclusion criteria

Exclusion criteria: Unemployment for more than 12 months at the time of cardiac rehabilitation; lack of informed consent for study participation; insufficient proficiency in German.
