Inclusion criteria
Inclusion criteria: Patients capable of giving consent; patients who are to be admitted as inpatients
Exclusion criteria
Exclusion criteria: Patients who are not to be admitted as inpatients; patients for whom a medication history cannot be taken; patients for with no pre-visit medication documentation available
