Inclusion criteria: Post-COVID disease; sufficient physical and mental capacity to participate

Exclusion criteria

Exclusion criteria: No German language skills sufficient for study participation; no consent to study participation.

Description of the feasibility, acceptance and added value of the new concept; implementation of the offer and acceptance by GP practices (target achieved with registration of 10 % (N=500) of all GP practices in Lower Saxony); description of the use of ViCoReK-NDS, conclusions about the need for services to improve the care of post-COVID patients by supporting primary care; quantitative documentation of the utilization of the offer (expectation: within one year approx. 300 single case consultations with family physicians or patient consultations expected (3-4/day)).	—
